A Longitudinal Evaluation of the Impacts of Integrated Care for Complex Populations (ICCP)
A longitudinal evaluation that integrated propensity scoring models with longitudinal multilevel models was implemented to estimate the impacts of integrated care for complex populations (ICCP). Individuals who were in the Senior Enhanced Care project were used as a comparison group. The evaluation was strongly informed by a realist evaluation framework that links context and program mechanisms with program impacts. The Evaluation Centre combined qualitative analysis with longitudinal multilevel models to link program processes to impacts. Strong impacts of integrated care on a range of health outcomes were obtained.
